7th Grade Parents
7th graders need a varivax vaccine unless
they had chicken pox disease. Hepatitis B immunization is also required; 3-dose child series
or 2-dose adolescent series if starting doses
after age 11.  
  
REQUIRED HEALTH EXAMINATIONS
by the State of Pennsylvania   

7th Grade
Dental Form is due in September.  Click on Forms.
6th Grade
Physical Exam Form is due in Sept. Click on Forms.
3rd Grade
Dental Form is due in September. Click on Forms. 
Kindergarten
Physical Exam & Dental Forms are due in Sept.

MEDICATIONS
 If your child receives medication, please remember we need new medication orders every school year. Click on Forms, scroll down to Health Room, click on Medication Permission.
If you change jobs, move to a new home or
change your cell phone number at
any time during the school year do not forget to
notify the nurse.  It is essential that our emergency card has the most up-to-date information on contacting you about your child!  
 
MEDICATIONS IN NURSE'S OFFICE
All medications must be sent to the Health Room.  Children are never to keep medication on their person such as allergy pills, any non-prescription or prescription medicine.  Cough drops must also be sent to the Office

ASTHMA OR ALLERGIES
If your child has food or other allergies or has bee sting allergies please fill out an Allergy Action Plan or Bee Sting Allergy Action Plan. 
If your child has Asthma 
please fill out an Asthma Action Plan for us.
